To confirm your identity, assess your eligibility or entitlement to be issued a certificate of identity or refugee travel document, the Department of Internal Affairs (the Department, we) will collect the following personal information from you:
- Full name
- City or town of birth
- Country of birth
- Citizenship or nationality
- Date of birth
- Gender
- Height
- Eye colour
- Photograph
- Parent names
- Spouse or civil union partner’s name.
If the following apply to your circumstances and you are applying with a certificate of identity and refugee travel document application form, we will collect:
- The names of your adoptive parents
- If you have changed your name by marriage, civil union, statutory declaration or deed poll.
- You will need to send us the original documents (not copies).
To contact you about your application, travel document, and to deliver the travel document to you, we will collect:
- Residential address (mandatory)
- Delivery address (if different to your residential address)
- Phone number (mandatory)
- Email address.
If you are applying with the certificate of identity and refugee travel document application form, and you are not a refugee, you must tell us why you cannot get a passport from your country of citizenship. You must send us:
- Your passport
- The travel document you used to enter New Zealand
- Any other travel documents you have been issued
- A letter from Immigration New Zealand confirming your residential or refugee status
- Your travel document with your New Zealand residence permit, returning resident’s visa, work permit visa or visitor’s visa
- A letter from your embassy explaining why they cannot issue you a passport.
To process your application and add the relevant endorsement label to support your use of the travel document, we collect:
- If you wear a head covering or headband in your application photo for medical or religious reasons
- If you are unable to sign
- If the application has been completed by someone else on your behalf.
If the application is completed for someone aged 15 years or older, we require consent from a legal guardian. For this purpose, we collect:
- The birth parent or legal guardian’s full name
- Their date of birth
- Their contact information (phone number, email and residential address)
- The applicant's birth certificate or guardianship order.
If you are applying with a certificate of identity or refugee travel document paper application form, you must provide a witness. We will collect the following details from the witness to confirm they meet our witness criteria, and may contact them if necessary to verify the applicant’s identity:
- Full name
- Date of birth
- New Zealand passport number
- Residential address
- Phone number
- The name they know the applicant by
- How long the witness has known the applicant.
If you are applying with a refugee travel document – quota refugee application form, Immigration New Zealand (INZ) will:
- receive your personal information to complete and submit your application to the Department
- check your information against their records before it is submitted.
