Name changes by deed poll or statutory declaration

Read this page carefully if you have changed your name by deed poll or statutory declaration and you want this name to appear in your passport.

Important: You are not able to renew your passport online if you have changed your name since your last passport was issued.

Details of your name changes are required on the application form

Name change registered in New Zealand

If you were born in New Zealand and changed your name in New Zealand or born overseas and changed your name in New Zealand after the 25 January 2009, you will not need to provide a name change document.

Name changes executed in the United Kingdom

If you wish to have a change of name that was executed in the United Kingdom in your New Zealand passport, you must provide the deed poll certificate (Statutory Declaration) that has been executed by a Solicitor of the High Court (or equivalent jurisdiction).

A Certificate from the United Kingdom Deed Poll Service or other provider, where the certificate has not been fully executed by a Solicitor (of the High Court), is not acceptable for a change of name in a New Zealand passport.

Name change registered overseas (outside New Zealand)

If you changed your name overseas, you will need to send in the original name change documents. Documents not written in English must be translated by a translation service approved by the Department of Internal Affairs.

You were born overseas and name change registered in New Zealand after 25 January 2009
  • No documents are required.
You were born overseas (outside New Zealand) and had your name change registered in New Zealand before 25 January 2009
  • Original name change documents are required.
You were born overseas (outside New Zealand) and changed your name overseas
  • Original name change documents are required. Documents not written in English must be translated by a translation service approved by the Department of Internal Affairs.
You had a Change of Name by Deed Poll Certificate (Statutory Declaration) executed in the United Kingdom
  • Original name change documents are required. Documents not written in English must be translated by a translation service approved by the Department of Internal Affairs.

Avoid delays

If you are required to send original documents that are not written in English, they must be translated by a translation service approved by the Department of Internal Affairs. We also accept official translations of documents into English by the issuing Government agency or a New Zealand diplomatic post. We will accept translations made by a court in the country of origin if they have been authenticated by that country's diplomatic post in New Zealand.
