Apply for an emergency travel document – contact and delivery details

Tell us your contact, delivery and emergency details

We need to know where we can reach you if we need to contact you about your application. You also need to tell us where your new passport is being sent after we have processed your application.

Application section 4 – Your contact and delivery details

Your contact details

  • Your home address is the place where you are currently living, do not give a post box, post bag or a ‘care of’ (c/-) address.
  • Make sure your contact details are correct as we will use these if we have questions about your application.

Your delivery address

  • Delivery arrangements should be made directly with the office issuing the emergency travel document.

Application section 10 – Your emergency contact

This information is optional but it’s a good idea to provide the details of someone who can be contacted in emergency situations.

We recommend that you nominate a person who is not likely to be travelling with you. The best plans can be upset by a number of problems. Some are impossible to predict or avoid, so by planning ahead and arming yourself with the right information, you can reduce the risks you might encounter overseas.

Before you travel, you should also go to safetravel.govt.nz and register your travel plans. While you’re there, check the travel advisories for countries you are travelling to.

When will we use your emergency contact?

  • If you have an accident, become sick or are involved in an incident while you are travelling overseas.
  • Passport offices overseas, New Zealand Embassies, High Commissions and Consulates can obtain your emergency information by request.
