This section provides some additional information to help you fill out your application form.

When you are applying for an adult passport you should complete the form yourself, if you are able to do so.

• Choosing a service level

  Examples of how to indicate on your form what service level you are using, including urgent and after-hours applications.

• Converting your height into centimetres

  You are required to provide your height in centimetres, not feet and inches. Here's a table you can use for the conversion.

• Getting help to complete your form

  If you cannot fill in this form yourself because you have a disability or language difficulty, someone else can do it for you.

• English translation services

  Documents not in English must be translated. Original documents must be sent in with the English translation.
