Apply for a child passport - previous documents

Details of previous travel documents help prove your child’s identity

The following section explains what information you are required to provide if your child has been issued other New Zealand travel documents in the past. Read the instructions carefully to confirm which sections of the form you need to fill in.

Application section 3 - Previous New Zealand passport or other New Zealand travel document

You must fill in this section if your child has ever had a travel document issued by the New Zealand Department of Internal Affairs. Travel documents include passports, emergency travel documents, certificates of identity or refugee travel documents.

Previous passport or travel document checklist

Updating the details of your child’s existing passport?
  • You need to refer to their most recent passport and tell us the passport number.
  • You don’t need to send us their previous passport, but it should not be used for travel while the application is being processed.
  • Once your child's new passport has been issued, the old passport will be cancelled and cannot be used for travel.
Has the child already been issued an Emergency Travel Document and is now getting their full replacement passport?
  • You must send us their emergency travel document – even if it is expired, OR
  • Fill in section 8 of the child passport application form and declare their child’s previous Emergency Travel Document lost.
Has your child at any time been issued a Certificate of Identity of Refugee Travel Document and you are now applying for their first New Zealand passport?
  • You must send us their existing Certificate of Identity or Refugee Travel Document - even if it is expired, OR
  • Fill in section 8 of the child passport application form and declare your child’s previous travel document lost.

Help us protect New Zealand's security

You must provide the document number from your child’s most recent passport or New Zealand travel document or declare the document lost or stolen. If the passport or travel document is lost or stolen:

  • a police report is required, AND
  • you must complete section 8 of the form to declare your child's passport lost.
  • We will then cancel the lost passport to protect your identity and New Zealand’s border security.

Application section 8 - Declaration for a lost or stolen passport or other travel document

Only fill in this section of the form if your child’s New Zealand passport or travel document has been lost or stolen.
  • You are required to provide a police report
  • You must explain how the document was lost or stolen in section 8 of the form.

Your child's application may be delayed

The loss or theft of a passport or travel document is a serious matter. Replacing your child’s passport may take longer, as we may need to make further enquiries before we can complete processing the application.

Declaration checklist

Have you given us details of how the document was stolen?
  • Include as many details as possible, including the address where you believe the loss or theft occurred.
  • If you need more space to provide further details, attach a separate piece of paper to your application form.
Do you know the details of the lost or stolen document?
  • Fill in the document number and tell us where and when the document was issued.
Have you checked all the details of your declaration?
  • The person completing the form must sign and date the bottom of page 5.
  • Don’t forget to sign the consent to issue a passport to a child at section 10 on the bottom of page 7.
