Renew your adult passport - applicant checklist and declaration (paper form)
Before you sign your application, use this checklist to help you avoid mistakes and delays
Check the following information carefully to make sure you have provided all the information we require.
Your previous passport
You are not required to send your New Zealand passport to us. Once your application has been received your previous New Zealand passport will be cancelled and cannot be used for travel.

Avoid delays
Check you have answered all the questions on the form which are related to you. We can only process your application if you complete the form accurately. If you miss out any details or get them incorrect, we may need to contact you and there may be a delay in the processing of your application. You must send the complete application form, even if some pages have not been filled in by you and there are empty fields.

Is your paper form complete?
Have you checked your application and ensured it has been correctly completed?
- Read each question again and make sure you have followed all of the instructions.
Have you written the correct payment amount for your application?
- Look down the table of fees and charges information if you are unsure.
- If you are paying by cheque or money order, make sure the amount is correct, all details are filled in and the amount is payable to 'the Department of Internal Affairs'.
Have you completed your identity referee's information correctly?
- Make sure you have their correct identity referee details, including their New Zealand passport number.
Have you included 2 identical colour photos that are no more than 6 months old?
- Check your photos are not damaged.
- Check you have written your name on the back of one of your photos.
Were you required to complete a statutory declaration at section 5 of your application?
- Make sure you have signed the statutory declaration.
Have your signed section 5 of the form?
- Read each of the statements shown in the declaration. Your passport application is a legal document.
Warning
Penalties apply if you make a false or misleading statement in an attempt to get a New Zealand passport.

Will you need to send any documents?
- No documents are required.
- You are not required to send your New Zealand passport to us. Once your application has been received your previous New Zealand passport will be cancelled and cannot be used for travel.

Completing section 5 - Applicant declaration
Avoid delays
Complete a statutory declaration if you now wear a head covering or you are no longer able to sign your name.
Head coverings
If you wear a head covering for religious or medical reasons, please tick this option. If you did not wear a head covering in your last passport, you will need to complete a separate Statutory Declaration form telling us why you now wear a head covering, and send this with your application.
- Make sure you have signed the statutory declaration.
- Attach the statutory declaration to your renewal form.
Signing the renewal declaration
Read each of the statements shown in the declaration.
You must sign the declaration if:
- you have filled in your application form yourself, or
- you can sign your own name, but someone else has filled in the renewal form for you.
If you are not able to sign your name:
- Tick the box 'I am unable to sign my name' on page 3.
- If you were able to sign your name in your most recent passport, you will need to have the person who filled in your renewal form complete a statutory declaration stating the reason why you are now unable to sign this application. Attach and send this declaration to your rewnewal form.
